Grantees will use this document as a guide for completing their GYO Annual Report. Link on how to submit the report is located in the instruction document.
The report is due no later than June 30, 2024.
Instructions for Completing the GYO Annual Report (Word Version)
Grantees will use this form to provide a public-facing description of the GYO program, its goals, outcomes and partners. Due 30 business days upon Grantee receipt of executed contract from ODE Procurement.
Grantees shall prepare and deliver Quarterly Expenditure Reports using Agency provided template. Submit expenditure reports to this GYO Smartsheet Link.
The variation in versions is based on the amount of Budgeted Line Items within each Budget Category.
Updated Expenditure Report Templates: Updated versions of the Expenditure Report Templates include columns after Quarter 4 and Quarter 8, to track any movement of funding that is 25% or less of the total award amount.
V.2 GYO (Short) Budget and Expenditures Reporting Template
For changes that exceed 25 percent of the Total Approved Budget: Grantees must submit a Budget Change Request Form to this 2023-25 GYO Budget Change Request Smartsheet Link
Changes to a budget category and/or line item that exceeds 25 percent of the Total Project Budget REQUIRE APPROVAL from the Grant Manager BEFORE any adjustment or payment can be made. While the EAC understands adjustments may be needed during the course of the grant cycle, your project was selected based on the application submitted and the criteria set forth in the RFA. Not all requests may be approved if significant changes are made that deviate from the Project Plan and Budget Table that were selected by the RFA selection committee.
Due to the delays with grant agreements, we have extended the due date for this first report to Friday, April 19. The report is guided by requirements set forth by the Legislature from the 2023 session and designed to collect information on GYO Program Enrollment and Completion.
